Ontario legal name change applications

Bavi assists with Ontario legal name change applications for adults and children. The process involves a Service Ontario application, supporting documents, and in some cases a publication requirement. We help clients complete the paperwork correctly and avoid common filing errors.

Who can apply

Adult and child name changes

  • Any adult resident of Ontario can apply to change their own legal name
  • Parents can apply to change the name of a minor child, with the other parent's consent in most cases
  • Name changes are governed by the Change of Name Act (Ontario)
  • Applicants must generally have been an Ontario resident for at least one year
  • Certain criminal history may affect eligibility — Bavi will advise at intake
